Care & Wellness Check-In Services
Professional, compassionate support and regular updates for your loved one.
About Our Services
Support Circle Care & Wellness was founded by healthcare workers who saw a growing need that often goes unnoticed: seniors and adults with disabilities deserve more time, attention, and meaningful connection than busy environments can always provide. Families, too, deserve reassurance—especially when distance, work, or life makes it hard to be present as often as they’d like.
We are not a traditional home care service. We do not provide nursing or PSW care. Instead, we offer compassionate, non-medical wellness check-ins for individuals living in long-term care homes, retirement residences, or living independently.
Our role is simple but powerful:
We act as your family’s eyes and ears, providing trusted, unbiased check-ins when you can't be there yourself.
-
A wellness check-in is a non-medical, supportive visit focused on the emotional, social, and environmental well-being of your loved one.
It is not nursing care or PSW care.
Instead, it’s a compassionate, observational visit that includes:Friendly conversation and connection
A check on mood, engagement, and comfort
Visual observation of the environment (cleanliness, organization, safety concerns)
Awareness of any changes in appearance or behaviour
Listening to any concerns your loved one expresses
A detailed report sent to the family afterward
Think of it as having a trusted set of eyes and ears—someone who notices the small things, supports your loved one emotionally, and keeps you informed.
-
Our Wellness Check-Ins are ideal for families who:
Live far away and want consistent updates
Have loved ones in long-term care, retirement living, or at home alone
Want a professional’s eyes on safety, mood, and daily routines
Are navigating transitions like a new care home or recovery period
Need a trusted advocate monitoring their loved one’s wellbeing
-
Each visit includes gentle, attentive observation of your loved one’s:
Mood and emotional well-being
Appearance, comfort, and general presentation
Living environment (cleanliness, safety concerns, overall state)
Social engagement and level of connection or withdrawal
Adjustment to their home or care setting
Any concerns they personally express that may be important for family to know
-
After every visit, families receive a clear, compassionate report summarizing:
How their loved one appeared during the check-in
Notable changes in mood, behavior, or environment
Any observations that may help families stay informed, proactive, and connected
-
Our process is simple, flexible, and designed around your family’s needs.
1. Choose Your Visit Frequency
We offer weekly, bi-weekly, and monthly visit options. Families can adjust the schedule at any time—whether they need more frequent check-ins during transitions or fewer visits once things stabilize.2. We Visit Your Loved One
A trained wellness support professional meets with your loved one in their home, retirement residence, or long-term care home. Each visit includes conversation, emotional support, and a gentle visual assessment of their overall well-being.3. You Receive a Detailed Update
After every visit, families receive a clear, unbiased wellness summary. This helps you stay connected, informed, and confident that your loved one is safe, comfortable, and supported—no matter where you are.
